Your Appointment

After you have submitted the secure on-line intake form and it has been determined that your medical condition(s) qualify you for a cannabis prescripton we can schedule you an appointment to see the doctor.

On your appiontment day please remember to bring:

1. Your Ontario photo health card or if you have an old health card without a picture then please bring it along with another piece of government issued photo identification with i.e. drivers liscense or passport.

2. Any relevant medical documents related to the conditions you are seeking cannabis for. If you would like Phytomedical to obtain this information on your behalf then we can send you the necessary release forms.

Please keep in mind that if you have been referred to Phytomedical by a doctor then we should already have this information.
