Step 1: Fill out the online intake form

This necessary information is used to help determine if you qualify for an appointment with the doctor to get a legal medical cannabis prescription.

Step 2: We will contact you

After we have received your intake form we will contact you about booking an appointment to see a licensed medical doctor in regards to using cannabis as medicine.

As a member with Phytomedical and a medical cannabis patient you are given the choice to access cannabis legally using one of the Health Canada approved licensed producers, or to grow your own medical cannabis through the Access to Cannabis for Medical Purposes Regulations (ACMPR) program.

Step 3: Book your appointment

If you pass the qualification process then we can move on to booking you an appointment.

Step 4: Pay the membership fee to reserve your appointment

You can reserve your appointment by paying the membership fee. We take payment fees ahead of time to ensure  people make it to their appointment. A lot of time is spent on administration and preparation of paperwork for each appointment and it is unfair to those who are still waiting for an appointment to have people no-showing.

After payment is received you will get a confirmation email from Phytomedical with your appointment date, time and location as well as a brief reminder of what you should bring with you to your appointment.

Please note: If due to unforeseen circumstances you are unable to make your agreed upon appointment time or date after you have already paid then please let us know as soon as possible and we will do our best to adjust your time or we will reschedule you on our next available day.

If for any reason the doctor decides not to prescribe you cannabis then you will be issued a full and complete refund as you will not be eligible for our cannabis counselling services. However, if you are prescribed cannabis and are unhappy with the amount you are prescribed by the doctor and refuse to accept it then you will not be entitled to a refund.
